HIV Prevalence Remains High Among Gay Men

In 2008, the prevalence of HIV was 19% among more than 8,000 men who have sex with men (MSM), the CDC reported.

And of those infected in the cohort, 44% were not aware of the fact, the agency reported in the Sept. 24 issue of Morbidity and Mortality Weekly Report.

Prevalence of infection was highest among blacks, at 28%, followed by Hispanics (18%), non-Hispanic whites (16%), and people who were multiracial or of other race (17%), the agency reported.

The findings come from data collected by the CDC's National HIV Behavioral Surveillance system, using anonymous interviews conducted at bars, clubs, and social organizations frequented by MSM.

The surveillance is conducted among MSM, injection-drug users, and heterosexuals at increased risk for HIV infection, with a different risk group studied every year. The first surveillance cycle among MSM was conducted in 15 metropolitan statistical areas during 2004-2005.

The 2008 survey was carried out in 21 such urban areas, which in 2006 had included about 60% of all urban U.S. AIDS cases, the CDC reported.

Men eligible for the interview -- 18 or older, living in the area, and able to complete the interview in Spanish or English -- were offered anonymous HIV testing, the opportunity to get results, and anonymous referral to care if appropriate.

All told, the CDC interviewed and tested 8,153 men, or 74% of those eligible. The median age was 32, and 44% were white, 25% were Hispanic, and 23% were black. The remainder were Asian, Native Hawaiian or Pacific Islander, American Indian or Alaska Native, and multiracial or other race, the agency reported.

A majority -- 66% -- had health insurance and 76% said they had visited a healthcare provider in the previous year.

Of the 1,562 men with HIV, 680 didn't know it, and the proportion of men who weren't aware of their infection was higher among younger men, reaching a high of 75% among those ages 18 and 19 and declining steadily to a low of 19% among those 50 and older.

Among blacks, 59% were not aware of their infection, while 46% of Hispanics and 26% of whites did not know they had HIV; among those younger than 30, blacks were most likely unaware they had HIV.

For comparison purposes, the CDC looked at the 2004-2005 surveillance report of MSM HIV prevalence. That report found an HIV prevalence of 26% among MSM and an infection unawareness rate of 48% in five urban areas -- Baltimore, Los Angeles, Miami, New York, and San Francisco.

When those areas were analyzed separately in the 2008 data, the agency found the numbers virtually unchanged with prevalence at 27% and 48% of HIV-positive participants unaware of their infection.

Among blacks, HIV prevalence was 40% and 63% were unaware of their infection. The corresponding rates in 2004-2005 were 46% and 67%, respectively.

The findings confirm that HIV prevalence remains high among MSM, the agency concluded, and show that racial and economic disparities remain.

But, the agency said, the analysis is subject to several limitations, including the fact that the survey was administered by an interviewer, so that positive HIV status might have been under-reported during the interview, which would inflate estimates of MSM unaware of infection.

They also noted that the analysis did not control for demographic differences in the samples from 2008 and 2004-2005, which might influence the findings.

And because the findings apply to men are who frequented certain places, most of which were bars and dance clubs, in urban areas with a high prevalence of AIDS, the results are not representative of all men who have sex with men and, in fact, a lower HIV prevalence of 11.8% has been reported among MSM in the general population.

Remember:  Boycott Target and Best Buy for donating money towards anti-gay political candidates/organizations and to Proposition 8 (California) to prevent gay marriage. The Formal Boycott began this past weekend. I stopped shopping these stores - even though I want a new washer/dryer and new clothes for the Fall.  I would not purchase "Blood Diamonds," nor should a person / organization be contributing funds to both Hitler / Nazi's and the Jews at the same time!   Obviously giving money to the Jews and not Hitler/Nazi's is the right thing to do.   Hence, these companies violated my trust and well-being.   They gave money to my enemy who would terminate me from my job, deny me housing and send me to a Internment camp.  Therefore, goodbye Target and Best Buy!  The CEO's of these companies have been insensitive to our reaction and do not even comprehend their actions!  What takes so long to "fire" these Neanderthals?
